Co-payments

All co-payments are due at the time of check-in.

Cancelled Appointments and Late Arrivals

We must be notified 24 hours in advance for cancelling any appointment, otherwise, a “No Show” fee of $50 will be charged. This fee is not covered by insurance.

In order to provide timely service to all of our patients, patients must arrive within 15 minutes of their appointment or it will be considered a “No Show” and a fee of $50 will be charged. If you are significantly late (over 15 minutes), we may try to work you back into the schedule, but we may have no other option but to offer an appointment to you with another provider or to reschedule another appointment for you.

No Walk-In Appointments

We do not accept walk-in appointments as this compromises patient flow and care. If a patient comes in without an appointment scheduled, we may triage the situation and determine whether the patient needs to be seen urgently. We will work the patient into our schedule if necessary. In these situations, we charge a $40 walk-in fee which is not covered by insurance and is due at the time of service.

Outstanding Balances

Outstanding balances for all family members must be paid prior to scheduled visits. If there is still a balance at time of check-in, you must pay your balance at that time in order to proceed with your office visit.

Collections

All past due accounts will be sent two statements and a letter of final notice. If payment has still not been made, a single phone call will be made in attempt to make payment arrangements. If no resolution can be agreed upon, the account will be sent to our collection agency.

Medical Records

No fee will be incurred for transfer of care to another provider. However, a $25 fee will be billed for copies of medical records for personal keeping. Please note, we need to receive requests two weeks in advance to complete the request.

Authorization to Treat Minors

We are unable to treat any minor (under 18 year old) without a parent or legal guardian consent. A minor may be treated in the presence of an adult other than the parent or legal guardian with proper written consent.

Returned Checks

A $25 charge will be billed for any returned checks.
