Please remember to bring your insurance card with you at every visit.
Please note that your insurance policy is your contract agreement with your insurance company. As such, you are responsible for knowing the terms and conditions of your own policy and it is not the responsibility of OC Children’s Medical Group to know the details. As a courtesy, we will submit claims on your behalf, but we will not know the exact details of what your insurance company will pay for until the claim has been processed. Therefore, if you have a dispute over the resulting billed amount, it is your responsibility to contact your insurance company. Please ensure you complete the following requirements in order to avoid being responsible for the full cost of the services rendered and to avoid submitting your claim on your own to your insurance company.
✓ Ensure our physicians are in your insurance network
✓ Ensure your insurance and contact information are up-to-date with our office
✓ Ensure your newborn has been added to your policy before their first appointment
✓ Ensure one of our physicians is assigned as your child’s primary care physician if you have HMO insurance
If we file your claim, you are responsible for the co-pay at the time of visit. If we are not a provider for your insurance or if you are not insured, you may be responsible for the entire charge at the time of service.
